Résumé
Afin d’assurer au patient des soins adaptés à son état et son rapatriement vers son pays d’origine et cela où qu’il se trouve dans le monde, deux types de transport sanitaire sont assurés : l’évacuation sanitaire urgente, lorsque le plateau technique est manifestement insuffisant sur place et que la balance bénéfice/risque lié au transport est très en faveur d’un transfert urgent, et des rapatriements moins urgents, une fois le patient stabilisé, pour lesquels un niveau de sécurité élevé est exigé. La réussite du rapatriement nécessite une articulation médicale et logistique importante. Elle repose sur l’évaluation la plus complète possible de l’état du patient. Elle tient compte des contraintes de la médecine aéronautique (hypoxie, expansion des gaz) et des contraintes logistiques et administratives, sans méconnaître le risque de transmission d’agents infectieux et d’importation de germes multirésistants. Si l’indication du rapatriement est retenue, il y a lieu de choisir le vecteur et de définir les modalités du transfert : avion spécial ou avion de ligne, avec aménagements particuliers (civière, oxygène). Cette démarche complexe impose anticipation, expertise et travail en équipe.
Abstract
To allow appropriate care to patient’s needs anywhere in the world and organize his/her return to home country, two types of transportation are provided: urgent aeromedical evacuation if patient’s needs exceed the capabilities of local facilities and more frequently, non-urgent transportation after stabilization abroad. The key in this procedure is the combination of a medical approach and logistic process. It requires the most accurate assessment of patient’s medical status. Aviation medicine issues including hypoxia and gas expansion, logistic and regulatory constraints should be taken into account. The risk of transmission and importation of multidrug resistant infectious agents represents one recent major concern. Transfer to home country can be performed by either air ambulance or commercial flight with specific arrangements (stretcher, oxygen). This process needs anticipation, expertise, and teamwork.
