Objectif
Évaluer la pertinence d’une médicalisation systématique du transfert vers l’unité neurovasculaire (UNV) de référence des patients présentant un accident vasculaire cérébral (AVC) ischémique téléthrombolysés dans un centre hospitalier (CH) de premier recours.
Matériel et méthodes
Étude rétrospective, monocentrique, observationnelle, sur 24 mois, en Franche-Comté. Évaluation du statut neurologique par les scores de Glasgow (GCS) et National Institute of Health Stroke Score (NIHSS). Évaluation des statuts respiratoire (SpO2, besoins en O2) et hémodynamique (pression artérielle, fréquence cardiaque [FC]) ainsi que de la nécessité de manoeuvres de réanimation aux différents temps de la prise en charge : thrombolyse, transport, arrivée à l’UNV.
Résultats
Soixante-dix-neuf patients âgés de 71 ± 15 ans ont été inclus. Le statut neurologique est stable ou amélioré chez 86 % des patients quelle que soit la valeur initiale du score de GCS ou NIHSS. Concernant le statut respiratoire et hémodynamique, 95 % des patients améliorent ou conservent une SpO2 supérieure à 91 %, et 96 % conservent ou améliorent leur pression artérielle systolique (PAS). Deux patients présentant initialement des troubles de la conscience ont été intubés, respectivement immédiatement après thrombolyse (NIHSS : 16) et au cours du transfert (NIHSS : 7). Aucune autre manoeuvre de réanimation n’a été nécessaire.
Conclusion
On observe une stabilité ou une amélioration du statut des patients au cours du transfert vers l’UNV de référence après thrombolyse. Les résultats de ce travail préliminaire nécessitent d’être confortés par une étude de plus grande ampleur, mais font discuter l’intérêt d’une médicalisation systématique de ces transferts.
Abstract
Objective
To assess the relevance of a systematic medicalization of transfer to the neurovascular (NVU) reference unit of patients with stroke who were telethrombolyzed before.
Materials and methods
Retrospective, monocentric, observational study, over 24 months, in Franche-Comte (France). Evaluation of neurological status, by the Glasgow score (GCS) and National Institute of Health Stroke Score (NIHSS). Assessment of respiratory status (SpO2; needs O2) and hemodynamic (arterial blood pressure, heart rate) as well as the need for medical resuscitation at different times of the transport (thrombolysis, transfer, arrival at the Regional University Hospital Centre).
Results
A total of 79 patients, aged 71 ± 15 years, were included. The neurological status is stable or improved in 86% of patients, regardless of baseline NIHSS score or GCS. For respiratory and hemodynamic status, 95% of patients improve or retain greater than 91% SpO2, and 96% maintain or improve their systolic blood pressure. Two patients initially presenting consciousness disorders were intubated, respectively, immediately after thrombolysis (initial NIHSS score 16) and during transfer (initial NIHSS 7). No other resuscitation maneuver was necessary.
Conclusion
There was a stability or an improvement of the status of patients during their transfer to the NVU reference after thrombolysis for stroke. The results of this preliminary work need to be confirmed by a study of greater magnitude but do discuss the interest of a systematic medicalization of these transfers.
