Résumé
La syphilis est une infection en pleine résurgence depuis les années 2000. La neurosyphilis est l’envahissement du système nerveux central par Treponema pallidum, se présentant la plupart du temps par une méningite aseptique. Nous décrivons ici le cas d’un homme de 77 ans présentant une méningite aseptique accompagnée de signes neurologiques déficitaires. Sa prise en charge est marquée par une aggravation initiale vers un coma nécessitant l’admission en réanimation malgré l’antibiothérapie probabiliste initiale. Le diagnostic de neurosyphilis est fait au décours, permettant une évolution favorable après adaptation des traitements. Les recommandations françaises actuelles de prise en charge des méningites ne semblent pas toujours adaptées à dépister les cas de neurosyphilis. Les cliniciens doivent avoir à l’esprit cette pathologie et la rechercher devant toute méningite aseptique de l’adulte afin de la traiter au plus vite.
Abstract
Syphilis is an infection in resurgence since the 2000’s. Neurosyphilis is the invasion of the central nervous system by Treponema pallidum, which in some cases appears in the form of aseptic meningitis. We describe the case of a 77-year-old man with aseptic meningitis accompanied by neurological deficits. Its management is marked by an initial worsening (coma) requiring ICU admission because of the inefficiency of the initial probabilistic antibiotic therapy. The diagnosis of neurosyphilis is done during early hospitalization in ICU, allowing recovery after antibiotics adjustment. Current French guidelines for treatment of meningitis seem inappropriate concerning neurosyphilis, leading to non-identification of the organism. Clinicians should take in mind this disease in the case of any aseptic meningitis to treat it as soon as possible.
