Objectif
Évaluer la conformité du conseil téléphonique en cas d’arrêt cardiaque (AC) au centre 15 (CRRA 15), au regard des recommandations 2010.
Matériel et méthodes
Étude prospective, observationnelle, monocentrique, réalisée après accord du comité de protection des personnes. Inclusion des AC extrahospitaliers reconnus à l’appel au 15. Conformité du conseil évaluée sur trois critères: demande d’installation de la victime en décubitus dorsal sur plan dur, compressions thoraciques au rythme de 100–120/min, amplitude de compression de 5–6 cm. Recueil des données: dossiers Centaure 15®, enregistrements téléphoniques du CRRA 15, dossier hospitalier pour la survie hospitalière.
Résultats
Analyse de 100 dossiers d’AC du 28 janvier au 4 avril 2013, dont 88 % survenus à domicile. Un conseil téléphonique était délivré dans 76 % des cas, par un assistant de régulation médicale dans 54 % des cas. Un conseil téléphonique conforme était prodigué dans 27 % des cas. Le conseil d’installation sur plan dur était systématiquement donné tandis que la fréquence et la profondeur de compression étaient erronées dans respectivement 61 et 72% des cas. La survenue de l’AC dans un lieu public équipé d’un défibrillateur (DAE) influençait de façon significative la qualité du conseil. Les facteurs influençant significativement la survie étaient la présence et l’utilisation du DAE ainsi que l’âge.
Conclusion
Vingt-sept pour cent des conseils téléphoniques dans le cadre d’un appel pour AC sont conformes aux recommandations. L’intégration aux logiciels de régulation de procédures pour le conseil en cas d’AC est une piste d’amélioration qui mériterait d’être testée au sein des CRRA 15.
Abstract
Objective
Assessment of compliance of phone advice delivered by a medical call center in case of cardiac arrest (CA) in regard to the last international recommendations.
Methods
A prospective, monocentric, and observational study was performed after approval by local ethic committee. Assessment of compliance was evaluated on the following criteria: installation of the victim on a hard surface, chest compressions at 100-120/min, and thoracic compression amplitude of 5 to 6cm. Data were collected from phone records and medical chart.
Results
From January 28 to April 4, 2013, 100 CA cases were studied. CA occurred in 88% of cases at home. A phone advice was delivered in 76% of cases, by an assistant of the call center in 54% of cases. The compliance of telephone advice delivered was 27%. The installation on a hard surface board was consistently delivered, while the frequency and depth of compression were wrong in 61% and 72% of cases, respectively. When CA occurred in a public place with automated external defibrillator (AED), it influenced significantly the quality of advice. AED presence and its use and age influenced significantly the survival.
Conclusion
Twenty-seven percent of telephone advices comply with the recommendations in case of CA. Integration of systematic procedures in the software is a track for improvement that should be tested within medical call center 15.
