Résumé

La fibrillation auriculaire est la plus fréquente des tachycardies supraventriculaires. La forme ECG typique associe (a) une absence d’onde P sinusale, (b) des auriculogrammes anarchiques, (c) des intervalles R-R chaotiques, (d) une réponse ventriculaire rapide et (e) des complexes QRS fins. Les formes atypiques sont nombreuses et parfois trompeuses, en rapport avec des anomalies de la conduction auriculoventriculaire et/ou intraventriculaire. Leur connaissance et une description précise de ces anomalies sont essentielles, car la prise en charge thérapeutique en dépend.

Abstract

Atrial fibrillation is the most common supraventricular tachycardia. The typical aspect on ECG combines (a) an absence of sinus P wave, (b) chaotic atrial depolarizations (c) chaotic R-R intervals, (d) a rapid ventricular response and (e) narrow QRS complexes. Atypical forms are numerous and sometimes misleading, secondary to atrioventricular and/or intraventricular conduction abnormalities. Their knowledge and an accurate description of these abnormalities are essential, because the treatment depends on it.
