Development and Psychometric Testing of a Scale to Measure Sexual Minority-Inclusivity in Sex Education (SMISES)

Abstract

Introduction

A reliable and valid measure of sexual minority (SM)-inclusivity is needed to support the development and evaluation of SM-inclusive sex education programs. We designed a 10-item scale that measures sexual minority-inclusivity in sex education and rigorously evaluated the scale’s psychometric properties.

Methods

The study team developed an initial set of items that was revised according to feedback received from 9 sex education experts and interviews with 12 SM youth. Next the items were administered as a survey to 186 SM youth ages 18–23 years. Psychometric analysis included internal consistency, dimensionality assessment, and item response theory (Rasch) calibration. Data was collected from October 2021 to February 2022.

Results

The 10-item scale (SMISES) measures key elements of SM-inclusivity in sex education: support, visibility, and relevant information. The SMISES demonstrated high content validity and strong psychometric properties, including unidimensionality, high internal consistency, and measurement precision. Rasch analysis suggested that receiving instruction about SM-related topics indicates a higher level of SM-inclusivity, while affirmation of SM-identities without instruction in SM-related topics reflects lower levels of SM-inclusivity.

Conclusions

This study is the first rigorous psychometric evaluation of a scale measuring SM-inclusivity in the sex education classroom. The SMISES is based on a clear, comprehensive definition of SM-inclusivity that was confirmed by expert review. Further study is required to examine the validity of the SMISES.

Policy Implications

The SMISES provides a promising measurement tool to inform the development and evaluation of SM-inclusive sex education programs. Rasch analysis is an important, underutilized methodology for improving and shortening scales.
