Résumé
Cette étude qualitative avait pour objectif de décrire et analyser les représentations de l’échec thérapeutique et les réponses médicales et sociales proposées par les professionnels de santé et par les patients suivis dans la cohorte ANRS 1215 au Sénégal de 1999 à 2010. L’histoire médicale des patients révèle que les échecs thérapeutiques sont liés à des situations complexes, multifactorielles, résultant de facteurs imputables aux patients (défaut d’observance liés à divers problèmes psycho-sociaux), mais également aux structures de soin (organisation du dispositif de soin, formation des professionnels de santé, disponibilité des examens biologiques et des médicaments adaptés). Ces causes sont reconnues aussi bien par les patients que par le personnel de soin. L’incidence de survenue des échecs thérapeutiques doit être considérée comme un témoin de la qualité des soins fournis. L’évolution de ces taux doit conduire à une réflexion sur le fonctionnement des dispositifs.
Abstract
The aim of this qualitative study was to describe and analyze representations of therapeutic failure and the medical and social responses proposed by health care professionals and patients receiving follow-up in the ANRS cohort 1215 in Senegal from 1999 to 2010. Patients’ medical histories show that therapeutic failures are related to complex multifactorial situations, resulting from factors attributable to patients (adherence failure related to various psychosocial problems) but also to health care structures (organization of the health system, training for health care professionals and availability of biological exams and appropriate drugs). Both patients and health care staff recognize these causes. The incidence of the onset of therapeutic failures should be regarded as an indicator of the quality of care provided. Further thought should be given to how well health care systems function when changes occur in these rates.
