Zusammenfassung
Das Nichtansprechen auf die Therapie ist im Alltag des Rheumatologen eine nicht seltene Situation. Dieser kann durch eine möglichst frühzeitige Diagnose und das Einsetzen der Therapie während des günstigen Zeitfensters etwa 15 bis 20 Wochen nach Krankheitsbeginn durch Behandlung mit „disease-modifying anti-rheumatic drugs“ (DMARD) vorgebeugt werden. Der im Beitrag beschriebene Fall eines Patienten mit seropositiver rheumatoider Arthritis zeigt, dass trotz anfänglicher häufiger Änderungen im Therapieansatz die Behandlung mit modernen Therapeutika erfolgreich zu einer Reduktion der Krankheitsaktivität führte. Therapieadhärenz sowie die regelmäßige Therapiemodifikation durch den Rheumatologen sind wichtige Voraussetzungen, damit echt refraktäre Krankheitsverläufe die Ausnahme werden.
Abstract
Lack of response to therapy is not uncommon in the everyday routine of a rheumatologist. This can be prevented by diagnosis as early as possible and the introduction therapy within a favourable timeframe approximately 15 to 20 weeks after the start of the illness by treatment with disease-modifying anti-rheumatic drugs (DMARD). The case described in the article of a patient with seropositive rheumatoid arthritis shows that initially there were frequent changes in therapeutic approach, but treatment with modern medications as well as competence have led to a low level of disease activity. Good adherence to therapy and the regular modification of therapy by the rheumatologist are important requirements to prevent intractable disease courses.
