Abstract
It is not so uncommon in clinical practice to reach the diagnosis of a chronic disease long after its onset, especially for rare diseases: in the case presented here the delay for the patient lasted not only years but decades. To us practitioners such cases are a challenge that we have to face with the willingness to question our diagnostic assumptions, favouring clinical data over predetermined interpretations. We deal therefore with cases that become paradigmatic for all our clinical work.
