Zusammenfassung
Injizierbare Filler sind ein wichtiger Bestandteil der ästhetischen Dermatologie. Sie dienen dazu, die Hautstruktur zu stärken, Fältchen/Falten und/oder Volumen auszugleichen. Vereinfacht lässt sich der Markt auf die abbaubaren Präparate, v. a. Hyaluronsäure (HA)-Präparate, Calcium-Hydroxylapatit und die Poly-L-Milchsäure (PLLA)-Präparate reduzieren. Fehlbehandlungen sind häufiger als unerwünschte Reaktionen und beruhen meistens auf Überkorrekturen in Arealen, wo aufgrund der Anatomie Fillerdepots leicht sichtbar werden. Ein typisches Beispiel hierfür ist die periokuläre Region, wo ein übermäßiges Volumen zur Korrektur von Hohlaugen bzw. feinen Fältchen leicht als „Wulst“ imponiert. Wenn HA-Präparate verwendet werden, lassen sich Überkorrekturen durch Hyaluronidase leicht und schnell korrigieren. Bei Nicht-HA-Fillern ist dies ungleich schwieriger. Ziel jedes Anwenders sollte es sein, Überkorrekturen zu vermeiden, d. h. Anwender sollten sich auf Indikationen begrenzen, die sie beherrschen und bei neuen Indikationen und neuen Fillern eher zu vorsichtig vorgehen. Eine Unterkorrektur ist leichter zu korrigieren als eine Überkorrektur.
Abstract
Injectable fillers are an important component of aesthetic dermatology. They are used to strengthen the skin structure, to smooth folds and wrinkles or to compensate for volume deficits. For simplification the market can be reduced to the degradable preparations, in particular hyaluronic acid (HA) preparations, calcium hydroxyapatite and poly-ʟ-lactide (PLLA) preparations. Unwanted aesthetic results due to mistreatment is more frequent than adverse reactions and is mostly due to overcorrection in a region where filler depots are easily visible due to the anatomy. A typical example is the periocular region where an excessive volume for correction of sunken eyes or fine wrinkles can easily be transformed into a “sausage”. When HA preparations are used overcorrection can easily and swiftly be rectified with hyaluronidase but with other preparations correction is more difficult and to varying degrees. The aim of every user should be to avoid overcorrection, i.e. users should limit themselves to indications which they are familiar with and be overcautious with new indications and new fillers. It is easier to rectify an undercorrection than an overcorrection.
