Zusammenfassung
Hintergrund
Ausfallzeiten, Einschränkung der Lebensqualität durch das Tragen unbequemer Kompressionskleidung und ein im Rahmen chirurgischer Fettreduktionen bestehendes Komplikationsrisiko sowie postoperative Schmerzen treiben die Suche nach seriösen nicht-invasiven Alternativen oder Ergänzungen voran.
Fragestellung
Effektivität und Einsetzbarkeit eines neuen Verfahrens im klinischen Alltag werden geprüft. TruSculpt™ von Cutera basiert auf der monopolaren Radiofrequenz und ist eine Methode zum lokalen, permanenten Fettabbau.
Material und Methode
TruSculpt™ produziert homogene Hitze gleichmäßig auf einer Fläche von 40 cm2 ohne „hot spots“, verteilt und erhält sie konstant über vier Minuten. Die im Gewebe erzielten Temperaturen führen in der Fettschicht zur Apoptose und damit dauerhaftem Fettabbau ohne Schädigung der darüber liegenden Haut. In einem Zeitraum von 18 Monaten wurden 72 Patienten im vier- bis sechswöchigen Intervall an unterschiedlichen Körperregionen behandelt. Die Evaluierung erfolgte durch Fotodokumentation, Selbst- und Fremdbeurteilung.
Ergebnisse und Diskussion
In allen Fällen konnte eine eindeutige Fettreduktion und Straffung festgestellt werden. Die Sicherheit des Verfahrens, die unkomplizierte Handhabung des Geräts, der Komfort für die Patienten und sichtbare positive Resultate machen truSculpt™ in unserem Zentrum zu einem wichtigen Element der nicht-invasiven Behandlung lokalisierter Adipositas. Das System eignet sich besonders gut zur Behandlung von Problemzonen wie dem oberen Rücken, Flanken, Oberarmen und Doppelkinn sowie der Optimierung nach vorangegangener Fettabsaugung.
Abstract
Background
The risk of adverse effects, inconvenient downtime and postoperative pain following liposuction support the demand for non-invasive alternatives capable of reducing focal adiposity. The wide range of existing devices produce variably appropriate results.
Objectives
Innovation based on the existing technology of monopolar radiofrequency suggests a feasibility of achieving the required results. Using the first device on the European market, initial experiences and clinical results are presented. The technology and its efficacy of action at the cellular level resulting in substantial fat reduction have been demonstrated in studies.
Material and methods
A temperature of 45 °C sustained for 3 min results in a 60 % loss of adipose cell viability, without irreversible damage to the skin. The extended duration of deep heating induces cell apoptosis and reduction of fatty tissue. Since January 2013 the truSculpt™ treatment has been administered to 72 patients on an outpatient basis and 2 treatment sessions within 6–8 weeks are required. First improvements are obtained 4–6 weeks after the first administration and final results are seen within a time frame of 2–3 months.
Conclusion
First experiences and results suggest that truSculpt™ is an appropriate option for problem zones in non-obese patients requiring maintenance of fat reduction and skin tightening. Especially treatment on areas like the neck, upper back, upper arms and flanks and the use for further improvement following liposuction showed excellent results.
