Zusammenfassung
Die autologe Fetttransplantation stellt heutzutage eine etablierte Behandlungsoption zur Rekonstruktion der Brust dar. Durch enthaltene Wachstumsfaktoren und adipöse mesenchymale Stammzellen besitzt Fettgewebe ein einzigartiges regeneratives Potenzial, welches jedoch auch mögliche Risiken birgt. So wächst in den letzten Jahren die Sorge um ein kanzerogenes Potenzial von Fetttransplantaten. In experimentellen Studien wurden zwar überzeugende Anhalte dafür geliefert, dass transplantiertes Fettgewebe das Wachstum bestehender maligner Tumorzellen fördert, doch konnten diese Beobachtungen in klinischen Studien für invasive Mammakarzinome nicht bestätigt werden. Bis zur Festlegung klarer Richtlinien, welche auf Erkenntnissen aus intensiven Forschungsbemühungen basieren müssen, sollte das Lipofilling der Brust durch erfahrene Ärzte nur nach sorgfältiger Statuserhebung der Patienten und bei vollkommener Tumorfreiheit erfolgen.
Abstract
Autologous fat grafting is a well-established therapy option for breast reconstruction. Growth factors and adipose-derived stem cells within the fat graft exhibit a unique regenerative potential; however, they also present a certain risk to patients and in recent years many publications have given rise to concern regarding the carcinogenicity of fat grafts. In experimental studies fat grafts promoted growth of existing malignant tumors. Interestingly, those observations could not be confirmed in clinical studies for invasive breast cancer patients. To create specific guidelines more basic research and extended clinical trials are necessary. Until then lipofilling of the breast should only be performed by experienced specialists in tumor-free patients and after careful assessment of the patient’s status.
