Microneedling for scar and collagen induction therapy

Zusammenfassung

Zur Behandlung von Narben und zur Rejuvenation stehen verschieden Methoden zur Verfügung. Im Unterschied zu allen ablativen Techniken bleibt bei der Behandlung mit Microneedling die schützende Epidermis erhalten. Indikationen sind u. a. Falten, Narben, Aknenarben und Schwangerschaftsstreifen. Die Behandlung erfolgt mit dem Dermaroller, einem sterilen Einmalinstrument, das als zugelassenes Medizinprodukt in unterschiedlichen Breiten und Nadellängen erhältlich ist. Verschiedene Roller unterschiedlicher Firmen sind erhältlich, allerdings sind nicht alle in Deutschland CE-geprüft.

Nach der Betäubung mit Anästhesiecremes wird die Haut gleichmäßig bis zu 16-mal in verschiedene Richtungen berollt und so eine gleichmäßige Struktur von < 200 Mikroverletzungen pro cm2 erzeugt. Narben werden perforiert und gelockert. Die Formation neuer Kollagenfasern sowie die klinische Glättung benötigen ein Jahr. Im Unterschied zu Laserbehandlungen sind beim Microneedling weniger Patienten von nachfolgender postinflammatorischer Hyperpigmentierung (PIH) betroffen.

Abstract

Various procedures have been established for the treatment of scars and for rejuvenation. Microneedling causes no damage to the epidermis, no wound and no down time other than fractional ablative lasers. Indications include scars, acne scars, stretch marks, wrinkles and rejuvenation. Microneedling devices are sterile disposable instruments registered as a medical device. They differ in size and length of needles. With the patient under topical anesthesia the devices with needles are rolled 16 times in various directions with mild pressure over the skin resulting in up to 200 pricks/cm2. Scars are perforated and smoothed. New collagen production and clinical improvement, however, needs up to 1 year to accumulate but postinflammatory hyperpigmentation (PIH) occurs far less frequently in microneedling than in laser treatment.
