Zusammenfassung
Filler sind Implantate, die der Volumensubstitution, z. B. von Weichteilen und Hautfalten, dienen. Einerseits sollen sie permanent wirken, andererseits möglichst keine Nebenwirkungen hervorrufen. Da es sich jedoch bei Fillern um Fremdkörper handelt, die invasiv eingebracht werden, sind sie mit nicht kalkulierbaren Risiken für die Zukunft behaftet. Bei resorbierbaren Materialien ist die Sicherheit höher, der Effekt jedoch begrenzt. Der vorliegende Beitrag gibt einen Überblick über die Suche nach dem „ultimativen“ Produkt und die historische Entwicklung der Filler – von frühen Versuchen mit Wachs, über Silikonöl und Kollagen bis zu den heute verwendeten Hyaluronsäure-basierten Fillern. Deren unterschiedliche Generationen, ihre Struktur und Funktionsweise sowie weitere innovative Entwicklungen auf Hyaluronsäure-Basis mit hoher Biokompatibilität werden beschrieben, gleichzeitig aber auch auf ihre Grenzen hingewiesen.
Abstract
Fillers are implants which serve as volume substitution for soft tissue and skin folds. They must function permanently and also cause no side effects. However, as fillers are foreign bodies which are invasively introduced, they are associated with incalculable risks for the future. The safety of resorbable materials is higher but their effect is limited. The present article gives a review of the search for the ultimate product and the historical development of fillers, from early attempts with wax through silicon oil and collagen up to the presently used hyaluronic acid-based fillers. The different generations, their structure and mode of function as well as further innovative developments based on hyaluronic acid with high biocompatibility are described as well as their limits.
