Prognose und Rekonstruktion bei oralem Plattenepithelkarzinom

Zusammenfassung

Das orale Plattenepithelkarzinom stellt eine weltweit relevante Erkrankung dar, die trotz des medizinischen Fortschritts weiter an Bedeutung gewinnt. Die Überlebensraten konnten während der letzten Jahrzehnte nicht deutlich gesteigert werden. Die Berücksichtigung der individuellen Prognosefaktoren und biologischen Merkmale des Tumors könnte der Schlüssel zur individuell optimierten Therapie sein, um sowohl das Überleben als auch die Lebensqualität für Patienten mit oralem Plattenepithelkarzinom zu verbessern. Neben den patientenindividuellen und tumorspezifischen Prognosefaktoren ist auch die Therapieentscheidung bedeutsam. Zur Heilung werden die aktuellen Therapieoptionen und zur Rehabilitation die Möglichkeiten der chirurgischen Rekonstruktion beschrieben. Hierzu stehen konventionelle oder mikrochirurgische Techniken unter Transplantation eines Fernlappens mit eigenem Gefäßanschluss zur Verfügung. Die dentale Implantologie ermöglicht auch bei Mundtrockenheit und großen Resektionsdefekten eine prothetische Rehabilitation. Dank der computergestützten Planung lassen sich dentale Implantate präzise inserieren.
