Résumé
La découverte d’une anomalie des organes génitaux àla naissance nécessite une prise en charge ciblée et rapide. Elle permet d’évaluer le sexe chromosomique de l’enfant et le risque vital éventuel d’insuffisance surrénalienne. Ensuite, des explorations biologiques et radiologiques plus ciblées permettront souvent d’approcher un diagnostic étiologique. Toutes les filles souffrant d’anomalies de la différenciation sexuelle (ADS) auront un diagnostic étiologique. Une cause ne sera retrouvée que chez seulement la moitié des ADS chez les enfants de sexe masculin. La question du choix du sexe se pose àla naissance pour un petit nombre d’enfants. Les ADS, qui sont des maladies (ou des variations ?) qui engagent le pronostic et la qualité de vie de l’enfant, mais aussi de l’adulte qu’il deviendra (identité, sexualité, fertilité) et de sa famille, nécessitent une prise en charge multidisciplinaire dans des centres experts.
Abstract
Disorders of sex development (DSD) refer to congenital conditions in which discordance exists between genetic, gonadal and anatomic sex in a newborn. First biological assessment evaluates chromosomal sex and a possible life-threatening adrenal insufficiency. Then, more targeted biological and radiological investigations will often approach an etiological diagnosis. All girls with DSD will have an etiological diagnosis, whereas a cause will be found in only half of 46,XY DSD. Gender assignment at birth will be questioned for a small number of children. DSD, which are disorders (or variations?), engaging the prognosis and quality of life of the child, but also the adult he will become (identity, sexuality, fertility) and his family, require a multidisciplinary approach in expert centers.
