Résumé

La génitoplastie masculinisante comprend plusieurs types d’interventions : cure d’un hypospadias, le plus souvent postérieur, abaissement testiculaire, correction d’anomalies scrotales. Elle concerne les garçons insuffisamment virilisés : disorder of sexual development (DSD), pour lesquels un bilan est toujours nécessaire (caryotype, bilan hormonal). Dans 50 % des cas, une étiologie précise est retrouvée : caryotype normal (46, XY DSD) associé à une anomalie de production, de métabolisme ou d’action des androgènes, ou au contraire, anomalie des chromosomes sexuels. Dans de nombreux cas, aucun diagnostic étiologique n’est établi. Sur le plan chirurgical, de multiples techniques existent, dont les principes sont exposés, ainsi que les résultats qui sont souvent décevants (taux élevé de complications, nécessité de reprises chirurgicales). Ceux-ci doivent être évalués à l’âge adulte, selon trois types de critères : cosmétiques, urinaires et sexuels.

Abstract

Masculinising genitoplasty involves several surgical procedures: cure of proximal hypospadias, treatment of undescended testicle and of scrotal anomalies. This concerns in particular cases of 46, XY DSD, sexual chromosomal DSD, or idiopathic insufficient virilisation (50% of cases). Several surgical techniques may be used, for which principles and results are reported.
