Résumé
L’objectif de l’étude est d’évaluer l’efficacité d’un laser de basse énergie sur les mucites radio induites chez des patients traités par radiothérapie à fortes doses, pour un carcinome indifférencié du nasopharynx.
Méthodes
C’est une étude descriptive, prospective concernant 22 patients, qui ont été répartis en deux groupes : le groupe 1 a bénéficié d’une prise en charge thérapeutique conventionnelle. Le groupe 2 a bénéficié d’un traitement laser, les patients ont été illuminés par 2 longueurs d’ondes combinées dans une même fibre optique (660 nm et 808 nm avec une même puissance de 150 mW), 4 fois par semaine durant toute la radiothérapie. Les critères d’évaluation ont concerné le grade de la mucite orale selon l’échelle de toxicité de l’OMS, ainsi que le délai de son apparition. L’intensité de la douleur est chiffrée chaque semaine sur une échelle visuelle analogique (EVA).
Résultats
Dans le groupe 1 tous les patients ont présenté une mucite de grades 3 ou 4. Dans le groupe laser, 7 patients ont eu une mucite de grade 2 et 1 patient a eu une mucite de grade 3. A la fin de la radiothérapie l’analyse a montré une différence significative dans le grade de la mucite entre les 2 groupes (p=0,0001).
Dans le groupe 1, les patients ont décrit la douleur comme insupportable et la chiffre à 10, dans le groupe 2, 1 patient relate une douleur à 6, et 5 patients une douleur à 5, il existe une différence significative entre les distributions sousjacentes des scores douleur EVA entre les 2 groupes (p=0,0001). La moyenne du délai d’apparition de la mucite a été observée à partir de j10 de l’irradiation dans le groupe 1, et à j15 dans le groupe 2 (p=0,0147).
Dans notre étude l’utilisation de deux longueurs d’ondes couplées dans une même fibre optique dont l’une en infrarouge, a vraisemblablement mené à une réduction significative de la sévérité de la mucite orale, de la douleur et du délai de son apparition.
Abstract
The objective of the study is to evaluate the effectiveness of a low-energy laser on radio-induced mucositis in patients treated with high-dose radiotherapy for undifferentiated nasopharyngeal carcinoma.
Method
It is a prospective descriptive study, involving 22 patients, who were divided into two groups: group 1 received a decision in conventional therapy, group 2 received laser treatment, patients were illuminated by two combined wavelengths (660 nm and 808 nm with a power of 150 mW), 4 times per week throughout the radiotherapy. The evaluation criteria concerned the grade of oral mucositis according to the scale of toxicity of WHO, and the time of its appearance. The intensity of pain is encrypted each week on a visual analogue scale (VAS).
Results
In group 1 all patients had mucositis grade 3 or 4. In the laser group, 7 patients had mucositis grade 2 and 1 patient had grade 3 mucositis. At the end of radiotherapy analysis showed a significant difference in the grade of mucositis between the two groups (p=0.0001).
In group 1, patients described the pain as unbearable and number 10, in group 2, 1 patient relates pain to 6, and 5 patients a pain to 5, there is a significant difference between the distributions underlying VAS pain scores between the two groups (p=0.0001). The average time to onset of mucositis was observed from j10 of irradiation in group 1, and j15 in group 2 (p=0.0147).
In our study the use of two wavelengths, one of which in infrared presumably led to a significant reduction in the severity of mucositis and pain and time of onset.
