Résumé

Objectif

L’apport de l’immunohistochimie (IHC) dans le diagnostic de certitude des angiosarcomes (AS).

Matériel et méthodes

Nous avons procédé à une double relecture de 30 lames étiquetées AS par deux médecins seniors (MS1 et MS2), dans le laboratoire d’anatomie pathologique 2, groupe hospitalier Pitié-Salpêtrière en France. Les blocs de paraffine provenaient du laboratoire d’anatomie pathologique du CHU Tokoin de Lomé, de 1995 à 2005.

Résultats

Le MS1 a pu poser le diagnostic avec certitude de 26 cas sur 30, soit 86,66 %, dont 25 AS et un sarcome de Kaposi (SK). Il a posé deux diagnostics erronés (un SK et un hémangiopericytome) et deux diagnostics incertains nécessitant l’utilisation de techniques complémentaires. Le MS2 a pu faire le diagnostic de certitude de toutes les lames en utilisant les techniques d’histologie conventionnelle associées à l’IHC avec une positivité des marqueurs endothéliaux. Un cas de SK qui avait été étiqueté AS à la première lecture a été mis en évidence à IHC.

Conclusion

Les formes bien différenciées des AS peuvent être diagnostiquées en histologie conventionnelle, mais l’apport de l’IHC s’avère déterminant dans le diagnostic de certitude des AS peu différenciés.

Abstract

Objective

The purpose of this study was to evaluate the contribution of immunohistochemistry compared with conventional histology in the diagnosis of angiosarcomas (AS).

Materials and methods

We conducted a rereading of 30 slides labeled angiosarcomas by two senior doctors (MS1 and MS2) in the Laboratory Pathology 2, Pitie-Salpetriere, France. Paraffin blocks were from the Laboratory Pathology CHU-Tokoin Lom, from 1995 to 2005.

Results

The MS1 has been the diagnosis of certainty of 26 cases out of 30 or (86.66%) twenty-five AS and Kaposi’s sarcoma. He asked two misdiagnoses (Kaposi’s sarcoma and hemangiopericytoma) and two uncertain diagnoses requiring the use of complementary techniques. MS2 was able to make the definitive diagnosis of all slides using the techniques of conventional histology associated with IHC positivity for endothelial markers. A case of Kaposi’s sarcoma who had been labeled AS in the first reading has been demonstrated through research of anti-HHV8 Ab.

Conclusion

The differentiated forms of AS can be diagnosed by conventional histology, but the contribution of IHC is determinant in the definitive diagnosis of poorly differentiated AS.
