Résumé
Les lymphomes osseux primitifs (LOP) sont rares. Ils représentent 5 % des tumeurs osseuses primitives et 1 % de l’ensemble des lymphomes. Il s’agit d’une étude rétrospective étalée sur quatre ans, concernant les cas des LOP colligés à l’Institut national d’oncologie au Maroc, entre janvier 2003 et décembre 2006. La médiane d’âge était de 44 ans, le délai moyen du diagnostic était de 11 mois. La localisation prédominante était le rachis dorsolombaire. Le traitement était basé sur la chimiothérapie et la radiothérapie. Une laminectomie a été réalisée dans les localisations rachidiennes menaçantes. Le protocole de chimiothérapie utilisé était le CHOP (cyclophosphamide, doxorubicine, vincristine et prednisone). Le suivi était variable avec des extrêmes allant d’un mois à trois ans. Dans la limite de ce suivi, aucune rechute n’a été décelée. Bien que les LOP soient des tumeurs agressives, le taux de guérison reste meilleur que le reste des tumeurs osseuses.
Abstract
Primary bone lymphomas (PBL) are rare. They account for 5% of the primary bone tumors and 1% of the whole of the lymphomas. This is a retrospective study over four years PBL cases gathered at the National Institute of Oncology in Morocco between January 2003 and December 2006. The median age was 44 years; the average time of diagnosis was 11 months. The predominant localization was the lumbar spine. Treatment was based on chemotherapy and radiotherapy. A laminectomy was conducted in the menacing spine localizations. The protocol of chemotherapy used was the CHOP. The follow-up varied, with extremes ranging from 1 month to 3 years. During this follow-up, no relapse was detected. Although the PBL are aggressive tumors, the prognosis is better than other bone tumors.
