Pure squamous cell carcinoma of the gallbladder

Résumé

Le carcinome épidermoïde primitif de la vésicule biliaire est une entité rare. Il représente moins de 2 % des carcinomes de la vésicule biliaire. Il est caractérisé par son envahissement locorégional rapide et son pronostic plus sombre que celui de l’adénocarcinome. En raison du retard diagnostique, ces tumeurs sont souvent diagnostiquées à un stade avancé. À travers quatre cas colligés, les auteurs discutent certaines difficultés diagnostiques et thérapeutiques de ces tumeurs. L’extension tumorale importante au moment du diagnostic et la présence d’un envahissement ganglionnaire constituent des facteurs péjoratifs dans la prise en charge du carcinome épidermoïde de la vésicule biliaire.

Abstract

Squamous cell carcinoma accounts for less than 2% of gallbladder carcinomas, and is characterised by an invasive growth, a reduced tendency for lymph node metastasis, an increased tendency for hepatic infiltration or liver metastasis and a poorer prognosis than adenocarcinoma. Because of diagnostic delays, this tumour is generally diagnosed in advanced stages. We describe 4 cases of pure squamous cell carcinoma; the authors discuss diagnosis and therapeutic difficulties of these tumours. In both histological types of gallbladder carcinoma, treatment depends on the grade of local and regional invasion and tumour spread at diagnosis.
