Samenvatting
Het Centraal Bureau voor de Statistiek (CBS) is met ingang van het statistiekjaar 2013 overgegaan op het automatisch coderen van doodsoorzaken. Hiermee wordt een betere internationale vergelijkbaarheid en grotere stabiliteit van de doodsoorzakenstatistiek in de tijd (trends) nagestreefd. De overgang op automatisch coderen brengt ingrijpende verschuivingen in de statistiek met zich mee. Ten opzichte van 2012 (handmatig gecodeerd) is er in 2013 (automatisch gecodeerd) een significante verandering in vóórkomen van doodsoorzaken bij negen van de zestien onderzochte ICD-10-hoofdstukken waargenomen. Verklaringen voor de waargenomen veranderingen worden gevonden in: 1) een andere codering van doodsoorzaken bij de invoering van het automatisch coderen, 2) het al of niet hanteren van ICD-10-updates, 3) een andere interpretatie van selectieprincipes voor de onderliggende doodsoorzaak, 4) het gebruik van additionele informatie bij het coderen van een doodsoorzakenformulier, 5) het (tijdelijk) ontbreken van correspondentie over onduidelijkheden met de invuller van het doodsoorzakenformulier, en 6) technische beperkingen van de programmatuur. De eerste drie factoren betekenen een aanpassing van de doodsoorzakenstatistiek aan internationale standaarden; de factoren vier tot zes zijn artefacten van de overgang op automatisch coderen en vereisen handmatige correctie. Gebruikers van de doodsoorzakenstatistiek moeten met deze factoren rekening houden bij de interpretatie van de cijfers.
Abstract
Since the year 2013, Statistics Netherlands uses an automated coding system (IRIS) for the routine production of cause of death data. A better international comparability and an improved stability of data in time (trends) were important motives for this change from manual to automated coding. The change of method caused changes in mortality statistics. There were significant changes in frequency of occurrence of underlying causes of death in nine of the sixteen ICD-10 chapters. Dementia/Alzheimer, infectious diseases and risk factors showed an increase as underlying cause of death at the expense of pneumonia and urinary tract infections. Explanation of the changes could be found in: 1) coding practice (different code for a medical entity at the introduction of automated coding), 2) ICD-10-updates (not used in manual coding, included in IRIS), 3) different selection of the underlying cause of death, 4) the use of additional information when coding a death certificate (not possible for IRIS), 5) the lack of a querying system for unclear death certificates (not available for IRIS), 6) technical restrictions of the IRIS software. The first three explanations signify an adaptation of death statistics to international standards. The explanations four, five and six are artefacts of automated coding and require manual interventions. Users of death statistics should be aware of this change of method and its consequences in order to interpret the data in a correct way.
