Samenvatting

Het zelfsturende vermogen van ouders wordt steeds belangrijker, zowel in het kader van de jeugdzorgtransitie als in het politieke kader van zelfmanagement en participatie. Blogboek is ontwikkeld om de eigen regie van ouders te versterken.

Abstract

Blogbook: empowers parents in giving direction to their child’s multi-disciplinary care process, with support of care professionals

Blogboek is an e-tool which allows parents to create and manage a personal health record online, and empowers them in giving direction to the multi-disciplinary care process when their child receives care from several professionals. Because Blogbook is created and managed by parents, its answers the current call for more empowerment and participation of patients, or parents as the legal representative of their child.
