Samenvatting
In brede kring geldt het reeds vele jaren als een vaststaand feit dat de verloskundige zorg in ons land buiten kantooruren tekortschiet. Voor ons is dit aanleiding geweest om op transparante wijze in kaart te brengen hoe de incidentie van de onbedoelde uitkomsten van baringen (perinatale sterfte en Apgar score < 7) zich het afgelopen decennium heeft ontwikkeld. Daarbij is gebruik gemaakt van een evaluatiemodel dat wij specifiek voor het Nederlandse verloskundig zorgsysteem hebben ontwikkeld. De gegevens voor deze studie zijn ontleend aan de Perinatale Registratie Nederland. De afgebakende onderzoekspopulatie is onderverdeeld in drie tijdsperioden: 2002-2004, 2005-2007 en 2008-2010. In de loop van deze jaren blijkt het perinatale sterftecijfer fors te zijn gedaald, dit in tegenstelling tot de incidentie van de Apgar score < 7. Daarbij is de daling van de perinatale sterfte bij de bevallingen buiten kantooruren aanzienlijk groter dan overdag. Met name in de laatste periode laat het perinatale sterftecijfer geen relevante verschillen tussen de dagdelen meer zien. Gebleken is dat het door ons beproefde evaluatiemodel een bruikbaar hulpmiddel is om de vele parallelle ontwikkelingen in het verloskundig zorgsysteem op transparante en consistente wijze in de tijd te volgen.

Design and application of a descriptive evaluation model for the obstetric care system.
Adverse outcomes of term childbirths in successive time periods
It is a widespread belief that obstetric care outside office hours falls short in the Netherlands—for us a reason to describe in a transparent way how the incidence of adverse outcomes of term childbirths (perinatal mortality and Apgar score < 7 ) has evolved over the past decade. To do so, we developed an evaluation model specifically geared to the Dutch obstetric care system. The data for this study was obtained from the Netherlands Perinatal Registry. The study population has been subdivided into three successive time periods : 2002–2004, 2005–2007 and 2008–2010. Over the years, the perinatal mortality rate appears to have greatly diminished in contrast to the incidence of Apgar score < 7. The decline of perinatal mortality in births outside office hours is considerably larger than during daytime. Especially in the most recent period there are hardly any differences in the relative incidence of perinatal mortality between the parts of the day. In addition, we found that our well tested evaluation model is a useful instrument in following the many parallel developments in the obstetric care system in a consistent and transparent way.
