Samenvatting
De partners in de proeftuin regio Rijnland hebben elkaar gevonden in de wens ook voor de toekomst goede en betaalbare gezondheidszorg in de regio Rijnland te behouden. Om dit te realiseren en kosten te beheersen kiezen de Rijnlandse partners voor de inrichting van een Accountable Care Organisatie (ACO), vertaald naar de Nederlandse situatie. Deze ACO wordt ingezet om de Triple Aim doelstellingen (betere zorg, betere gezondheid en lagere kosten) te bewerkstelligen.
