Samenvatting
Een mallet finger is een gebogen stand van de eindfalanx van een vinger, doordat de pees van de m. extensor digitorum communis is geruptureerd ter hoogte van de eindfalanx, als gevolg van een direct trauma op de uitgestrekte vinger. Het is een veel voorkomende aandoening, waar zowel huisarts, spoedarts, chirurg, orthopeed als plastisch chirurg regelmatig mee te maken krijgt. Een ossale mallet finger is een avulsiefractuur van de distale falanx ter plaatse van de insertie van de extensorpees. Hoewel de primaire behandeling van een tendinogene mallet finger vrijwel altijd conservatief is, vindt operatieve behandeling in het algemeen plaats bij een ossale mallet finger waarbij meer dan een derde van het gewrichtsoppervlak is aangedaan en waarbij al dan niet volaire subluxatie van de eindfalanx is opgetreden. Het nadeel van veel gangbare operatietechnieken is vaak de langdurige immobilisatie postoperatief. Wij beschrijven de minihaakplaatosteosynthese, een techniek waarbij vroege mobilisatie en een snel functioneel herstel mogelijk zijn.
Abstract
A malletfinger is a rupture of the extensor digitorum tendon at the distal phalanx caused by direct trauma on the extended finger. This condition is seen frequently by general practioners, emergency doctors, orthopedic surgeons, plastic surgeons as well as general surgeons. A bony malletfinger is defined as an avulsionfracture of the dorsal distal phalanx. Although the treatment of a malletfinger is conservative in most cases, surgical treatment may be considered in a bony malletfinger when more than one third of the articulating surface is damaged and whether or not volar subluxation of the distal phalanx occured. The disadvantage of many operative techniques is immobilisation for a long time. We describe an operating technique where early mobilisation is possible: the hookplate fixation osteosynthesis.
