Samenvatting
Doel
Inzicht krijgen in de nauwkeurigheid van in- en exclusie van traumapatiënten in een regionale traumaregistratie op basis van ziekenhuisdata.
Design
Pilotonderzoek in twee level 2-ziekenhuizen in de acute zorg regio Zwolle. In het ene ziekenhuis is het proces voor de in- en exclusie van patiënten geautomatiseerd en het andere ziekenhuis in- en excludeert patiënten handmatig.
Methode
Aan de hand van de door het Landelijk Netwerk Acute Zorg (LNAZ) opgestelde specifieke criteria voor in- en exclusie van patiënten in de traumaregistratie zijn alle patiënten die de Spoedeisende Hulp (SEH) van een van de twee ziekenhuizen bezochten, in december 2013 herbeoordeeld. Het totaal aantal door de onderzoeker geïncludeerde patiënten is vergeleken met het aantal geregistreerde patiënten in de landelijke traumaregistratie (LTR). Sensitiviteit en specificiteit zijn berekend.
Resultaten
In het ziekenhuis met het geautomatiseerde systeem bleek 57,1% van de door de onderzoeker geïncludeerde patiënten te ontbreken in de LTR. De sensitiviteit was 42,9% en indiceert een onnauwkeurige inclusie van patiënten. De specificiteit in dit ziekenhuis was 99,9% en geeft aan dat de exclusie nauwkeurig is. In het ziekenhuis waar patiënten handmatig worden geïncludeerd en geëxcludeerd, was de sensitiviteit 93,8% en de specificiteit 99,6%. Dit indiceert dat zowel de inals de exclusie van patiënten tamelijk nauwkeurig is.
Conclusie
Er is grote variatie in nauwkeurigheid van in- en exclusie van patiënten voor de landelijke traumaregistratie in twee ziekenhuizen in de regio Zwolle. De oorzaak lijkt te liggen in de inrichting van het geautomatiseerde systeem en de gebruikte query’s en algoritmen.
Abstract
Objective
Examine the accordance of the amount of included and excluded trauma patients in a Dutch regional trauma registry with hospital data. Design. Pilot study in two level 2 hospitals in the Zwolle region in the Netherlands. One of the hospitals uses a computerized system for in- and exclusion of patients and the other hospital includes and excludes patients by hand.
Method
By using predefined criteria of Landelijk Netwerk Acute Zorg (LNAZ) for inclusion and exclusion of patients in the trauma registry, all patients visiting the emergency departments of both hospitals in December 2013 were evaluated by the researcher. The resulting number of included patients was compared with included patients in the trauma registry (landelijke traumaregistratie; LTR). Sensitivity and specificity were calculated.
Results
Of all 84 patients included by the researcher in the hospital which uses a computerized system 57.1% was missing in the LTR. The sensitivity was 42.9%, showing an inaccurate inclusion of patients. The specificity is 99.9%, indicating a highly accurate exclusion system. The researcher included 65 patients in the hospital, which includes patients manually. The same amount of patients was included in the LTR, but four patients were deviant. This resulted in a sensitivity of 93.8% and a specificity of 99.6%. This indicates that both inclusion and exclusion systems were fairly accurate.
Conclusion
There is large variation in accuracy of inclusion and exclusion of patients in LTR between two hospitals in a Dutch trauma region. A clear structural cause seems to be located in the IT-infrastructure and the creation of queries and algorithms.
