Samenvatting
Screening van ziekenhuismedewerkers (ZHW’s) op tuberculose is onderdeel van een integraal infectiepreventiebeleid om besmetting met Mycobacterium tuberculosis in ziekenhuizen te voorkomen. In 2013 verscheen de herziene richtlijn ‘Tuberculosescreeningsbeleid ziekenhuismedewerkers’. De screening is voortaan gericht op vaststellen van latente tuberculose-infectie (LTBI), waarvoor de tuberculinehuidtest (THT) en/of de interferon-gamma release assay (IGRA) worden gebruikt, ook na BCG-vaccinatie. Een thoraxfoto wordt op indicatie verricht. Screening bij indiensttreding wordt beperkt tot ZHW’s die recent kunnen zijn blootgesteld, met name ZHW’s die korter dan 2 jaar afkomstig zijn uit een endemisch land of die daar recent en langer dan 4 weken in een ziekenhuis hebben gewerkt. Periodieke screening wordt alleen nog aanbevolen voor ZHW’s die werken op risicoafdelingen in ziekenhuizen waar gemiddeld meer dan 5 tuberculosepatiënten worden gediagnostiseerd. In de overige, laagrisico ziekenhuizen wordt volstaan met contactonderzoek: screening op infectie na bekende blootstelling volgens het ringprincipe. Met dit advies kunnen bedrijfsartsen van ziekenhuizen hun screeningsbeleid ten behoeve van medewerkers herzien, met inachtneming van de specifieke risicosetting van het betreffende ziekenhuis.
Abstract
The tuberculosis screening of hospital staff is part of a comprehensive programme aimed to prevent hospital workers developing tuberculosis. In 2013 a revised guideline on tuberculosis screening of hospital staff was released. Screening is now focused on identifying hospital workers with latent tuberculosis infection. The test method is the tuberculin skin test and/or an Interferon Gamma Release Assay (IGRA). BCG vaccination does not influence test methodology anymore. A chest X-ray is only rarely performed for screening. Screening at the start of employment is limited to hospital workers with potential recent exposure, particularly employees who arrived in the Netherlands less than two years ago from countries where tuberculosis is endemic, or who recently worked in a hospital in such a country for longer than four weeks. Periodic screening is only recommended for hospital workers of high-risk departments of hospitals with more than five patients annually. In other, low-risk, hospitals employees are screened after known exposure, following the ring principle. Occupational health physicians can use the new guideline to revise the screening policy for health care workers, taking into account the specific risk profile of the hospital concerned.
