Gezondheidsklachten, werkvermogen en vastgestelde beperkingen van werknemers met kanker

Samenvatting

Beoordeling van de belastbaarheid van werknemers met kanker kan moeilijk zijn en de uitkomst kan tot discussie leiden. Deze studie onderzoekt de relaties van zelfgerapporteerde gezondheidsklachten en zelfgerapporteerd werkvermogen van werknemers met kanker, met de beperkingen zoals vastgesteld door de verzekeringsarts (FML) bij eerste einde wachttijd WIA-beoordeling. Via een vragenlijst is informatie over achtergrondkenmerken, zelfgerapporteerde gezondheidsklachten en zelfgerapporteerd werkvermogen verkregen. De respons was 52% (n=662); bij 364 cliënten was ook een FML ingevuld. Bivariate relaties tussen meer zelfgerapporteerde gezondheidsklachten, respectievelijk geringer zelfgerapporteerd werkvermogen en meer beperkingen volgens de FML konden worden aangetoond. Multivariaat bleken zelfgerapporteerd sociaal-, rol- en emotioneel functioneren (mogelijk van belang voor participatie van cliënten) van de vragenlijst EORTCQLQ- C30, niet gerelateerd aan de vastgestelde psychische FMLbeperkingen. Daarom wordt aangeraden dat verzekeringsartsen in de uitvoeringspraktijk deze onderdelen van de EORTC-QLQ-C30 gebruiken als ondersteuning bij de WIA-beoordeling en met name bij het participatieadvies van werknemers met kanker.

Summary

Health complaints, work ability and assessed limitations of emplovees with cancer

Long-term complaints of (former)cancer patients influence their daily functioning, including their work participation. Employees with oncological disorders constitute an important section of the disability benefits claimants. Their numbers are expected to increase.

The assessment of the functional abilities of cancer survivors applying for a work disability benefit is difficult, and may lead to discussions. This study examines the associations of the self-reported health status and work ability of cancer survivors with their functional limitations (physical, mental and working hours) as assessed by an insurance physician at the first qualifying period. Data on socio-demographics, selfreported health status and self-reported work ability of cancer survivors were obtained with a questionnaire using existing reliable scales and the Work Ability Index. The response to the questionnaire was 52% (n=662); 484 cancer survivors were included in the study with 364 completed functional abilities lists.

The association of poor self-reported health status and limited work ability with multiple functional limitations, as assessed by an insurance physician, could be confirmed. In a multivariate analysis the selfreported social functioning, role functioning and emotional functioning (characteristics probably important for work participation), did not relate to the assessed mental limitations. It is suggested that insurance physicians use parts of the EORTC-QLQ-C30 questionnaire to support the assessment of functional abilities of cancer survivors and especially to give participation advice.
