Training in toepassing van het protocol Depressieve stoornis en veranderingen in de uitkomsten van de beoordeling van de belastbaarheid

SAMENVATTING

We bestudeerden het effect van een training in de toepassing van het protocol Depressieve stoornis op de beoordeling door verzekeringsartsen van de belastbaarheid van cliënten met een depressie. Verzekeringsartsen namen deel aan een gerandomiseerd gecontroleerd onderzoek: de interventiegroep (IG) kreeg de training, de controlegroep (CG) niet. De IG moest voorafgaande aan de training twee videocasussen van beoordelingsgesprekken met cliënten scoren volgens de functionele mogelijkhedenlijst en twee andere na de training. De CG scoorde alle vier videocasussen zonder training. Uitkomstmaten waren: aantal en ernst van de beperkingen, en de interbeoordelaarsbetrouwbaarheid. De training leidde tot een grotere interbeoordelaarsbetrouwbaarheid bij de IG dan bij de CG, zij het niet significant. Ook was er een tendens dat er meer beperkingen werden gesteld door de IG dan door de CG. Of en in welke mate zo’n toename van beperkingen ook zal leiden tot een hogere arbeidsongeschiktheidklasse zal verder onderzoek moeten uitwijzen.

Training the implementation of the protocol ‘Depressive disorder’ and the changes in the outcomes of disability assessments

We report on the effects of training the implementation of the protocol Depressive Disorder on insurance physicians’ (IPs) disability assessments of clients suffering depression. In 2009 we conducted a randomized controlled trial for IPs in which we compared an intervention group (IG, 21 IPs) with a control group (CG, 19 IPs). Before the training session the IG group scored the List of Functional Abilities (LFA) for work limitations of the clients presented in two video recordings of incapacity assessment interviews. A further two followed the training. The CG received no training. The outcomes were the mean sum score relating to the number and severity of the incapacities and the intraclass correlation of the scores. The training led to a greater inter-rater reliability in the IG than in the CG. However, this difference was not significant. The mean sum score tended to be higher in the IG than in the CG. Whether this higher number of limitations might also lead to a higher disability allowance, requires further investigation.
