SAMENVATTING
Onderwerp: Het terugdringen van langdurig ziekteverzuim.
Doel: Onderzoeken van de effectiviteit van gestructureerde vroegtijdige begeleiding onder medewerkers met verhoogd risico op langdurig toekomstig ziekteverzuim, in het voorkomen en/of verminderen van ziekteverzuim. Het experiment was gericht op de timing van de interventie, d.w.z. behandeling voordat ziekteverzuim daadwerkelijk plaatsvindt.
Soort onderzoek: In deze RCT werden medewerkers met een verhoogd risico op langdurig ziekteverzuim geselecteerd op basis van een screeningsvragenlijst. 299 personen met een verhoogd risico op langdurig ziekteverzuim in de toekomst werden willekeurig toegewezen aan een experimentele groep (n=147) of een controlegroep (n=152). De experimentele groep kreeg vroegtijdige begeleiding van de bedrijfsarts, in sommige gevallen gevolgd door een gerichte interventie. De controlegroep ontving gebruikelijke zorg (‘care as usual’).
Resultaten: Een aangepaste intention-to-treat-analyse liet significante verschillen (p=0,007) zien in totale ziekteverzuimduur over een jaar follow- up tussen de experimentele (gemiddelde 18,98; SD 29,50) en controlegroep (gemiddelde 31,13; SD 55,47).
Conclusie: Vroegtijdige begeleiding door de bedrijfsarts van medewerkers met een verhoogd risico op langdurig toekomstig ziekteverzuim draagt bij aan het verminderen van totaal ziekteverzuim.
SUMMARY
Structured early consultation with an occupational physician reduces sickness absence among office workers with an increased risk of longterm sickness absence. A randomized controlled study
Objective: To examine the efficacy of structured early consultations for employees at high risk of future long-term sickness absence, in the prevention and/or reduction of sickness absence. The focus of the experiment was on the timing of the intervention, that is, treatment before sickness absence actually occurs.
Methods: In this RCT, bank employees at high risk of long-term sickness absence were selected based on responses to a screening questionnaire developed especially for office workers: the ‘Balansmeter’. 299 Subjects at risk of future long-term sickness absence were randomized in an experimental group (n=147) and a control group (n=152). Subjects in the experimental group received a structured early consult with the occupational physician, in some cases followed by targeted intervention. The control group received care as usual.
Results: A modified intention-to-treat analysis revealed significant differences (p=0.007) in total sickness absence duration over a one year follow-up between the experimental group (mean 18.98; SD 29.50) and the control group (mean 31.13; SD 55.47).
Conclusion: Structured early consultation with an occupational physician for employees at high risk of future long-term sickness absence is successful in reducing total sickness absence.
