Samenvatting
Psychotherapie werkt, maar lang niet voor iedereen. Om de effectiviteit van psychotherapie verder te kunnen verhogen, zijn drie routes denkbaar: nieuwe therapieën ontwikkelen, inzicht in de onderliggende werkingsmechanismen en onderzoek naar het profiel van patiënten die optimaal profijt hebben van een bepaalde vorm van therapie. Nieuwe vormen van psychotherapie hebben echter zelden tot een hogere effectiviteit geleid. Over de werkingsmechanismen van psychotherapie wordt al decennialang veel beweerd, maar er is weinig aangetoond. Ons onderzoek laat zien hoe moeilijk het is vast te stellen welke processen nodig zijn voor herstel tijdens de behandeling. De meest aannemelijke oorzaak daarvan is diversiteit: verschillende patiënten ondergaan verschillende veranderprocessen, waardoor van een algemeen patroon in alle patiënten geen sprake is. Efficiënter is het waarschijnlijk om te onderzoeken welke patiëntfactoren voorspellen of iemand goed of slecht reageert op een bepaalde vorm van therapie; een aanpak die bekend staat als personalized medicine. Wij ontwikkelden een multivariate voorspellingsmodel waarmee voor individuele patiënten kan worden uitgerekend wat hun voorspelde eindscore is bij twee even effectieve vormen van therapie (cognitieve therapie en interpersoonlijke therapie) en hoe groot het voorspelde verschil tussen beide therapieën is. Uit dit onderzoek blijkt dat er een grote groep mensen is voor wie het een groot verschil maakt welke therapie ze krijgen.
