Elektroconvulsietherapie (ECT 1) verbetert vaak het leven van patiënten die lijden aan ernstige, invaliderende en/of levensbedreigende psychiatrische aandoeningen zoals depressie en catatonie. Het is al decennialang bekend dat het effect van ECT afhankelijk is van de opgewekte gegeneraliseerde insultactiviteit in de hersenen. Het menselijk brein is echter gemaakt om cerebrale ontregeling met verspreiding van ongecontroleerde, overgesynchroniseerde, neuronale elektrische signalen te voorkomen. Daardoor ontstaat insultactiviteit alleen als de elektrische stimulus in staat is de zogenaamde prikkeldrempel (PD) te overstijgen. De mechanismen die ervoor zorgen dat een insult wordt uitgelokt, zich verspreidt en weer wordt onderdrukt, kunnen ons meer leren over het functioneren van de hersenen en over de pathofysiologie van ernstige ziekten zoals depressie en epilepsie.
Abstract
These studies examine a technical detail of electroconvulsive therapy (ECT), an effective and safe treatment for mood disorders and some other – sometimes life-threatening – illnesses. To become effective, a seizure has to be elicited by overcoming the brain’s seizure threshold (ST). Firstly, the literature was examined on (hypothetical) determinants of the ST. Secondly, a prospective observational study was conducted, showing higher age and bilateral electrode placement to be of predictive value for higher ST. Taking these variables also into account, an MRI study revealed that a higher volume of cerebrospinal fluid predicted a higher ST. Moreover, presence of psychotic depression and previous ECT predicted more positive outcome of ECT. Bipolar depression predicted less adverse cognitive effects, and using concomitant antipsychotics during the course of ECT predicted more adverse cognitive effects. Finally, more specific ECT patient groups were examined retrospectively, showing (among others) that malignant catatonic patients may recover better in case of those patients with more autonomic dysfunctions and when ECT was performed on a daily basis. Moreover, patients treated with continuation ECT showed longer duration of seizure activity, if the time-interval between ECT sessions was longer.
Treating patients with ECT, and subsequent scientific research, is an exciting subject matter.

Bij ECT kan de elektrische dosis op verschillende manieren worden gekozen. Gebruikelijke methoden zijn: a) het geven van een standaard hoge dosis (bijvoorbeeld 400 mC), of b) de helft van de leeftijd als percentage van de maximale output van het ECT-apparaat (504 mC), of c) een individueel bepaalde dosis afhankelijk van de bij de eerste ECT-sessie gemeten PD; dit zijn, respectievelijk, de zogenaamde ‘vaste-hogedosis’-, ‘halve-leeftijd’- en de ‘dosistitratie’methode.

1 Bij een ECT-kuur wordt twee maal per week, gedurende meestal drie tot twaalf weken, onder een kortdurende narcose met spierverslapping, door middel van metalen plaatjes aan de zijkant van het hoofd van de patiënt (deze zogenaamde elektroden kunnen zowel beiderzijds ter hoogte van de slaap [bilateraal; BL] als alleen aan de rechter zijde [rechts unilateraal; RUL] worden geplaatst), gedurende enkele seconden een elektrische stimulus gegeven, waarmee epileptische activiteit in de hersenen wordt opgewekt. Dit is heilzaam bij ernstige (psychotische) depressies, onbehandelbare manieën, psychoses, catatonieën, maligne neuroleptica syndroom, en delier.
