Samenvatting
Appendicitis is een veelvoorkomend ziektebeeld bij kinderen en zij presenteren zich vaker dan volwassenen met een geperforeerde appendicitis. In de praktijk is er weinig eenduidigheid in beleid wat betreft de duur van de postoperatieve behandeling met antibiotica na geperforeerde appendicitis. Het doel van dit onderzoek is na te gaan of er de afgelopen jaren gerandomiseerde studies zijn uitgevoerd die een vastgestelde behandelduur met intraveneuze antibiotica vergeleken met een eerdere overschakeling op orale antibiotica bij kinderen met geperforeerde appendicitis. Er werden twee gerandomiseerde gecontroleerde studies gevonden en één retrospectieve studie. In deze studies werd overgestapt naar orale antibiotica indien het kind orale intake verdroeg. In twee van de drie studies was een tweede voorwaarde dat het kind koortsvrij was. Als uitkomstmaat werd onder andere gekeken naar postoperatieve abcesvorming en de opnameduur. In geen van de drie studies werd een significant verschil in postoperatieve abcesvorming aangetoond tussen beide groepen.Wel werd in één RCT en in de retrospectieve studie een significant verschil in opnameduur aangetoond. Op basis van deze resultaten lijkt het veilig om, in plaats van een vastgestelde duur van de intraveneuze antibiotische behandeling aan te houden, eerder over te stappen op orale toediening.
Summary
Perforated appendicitis is a common condition in children, for which intravenous antibiotics are standard medical management after appendectomy. However, duration and route of antibiotic administration vary among several centers. The objective of this study was to investigate whether there are randomized clinical trials in which the effects of treatment with long-term intravenous antibiotics are compared with short-term intravenous antibiotic therapy and conversion to oral antibiotics. Two randomized clinical trials were found and one retrospective study. In these studies, conversion to oral antibiotics was achieved when the patient was able to tolerate a regular diet. In two of the three studies, the patient also had to be afebrile. Outcome measures were postoperative abscess formation and duration of hospitalization. All three studies did not find a significant difference in rate of postoperative abscess formation. In two studies significant difference in duration of hospitalization was observed. According to these results, completing the course of antibiotics orally can be achieved safely.
