Samenvatting
Twee jonge kinderen presenteren zich binnen enkele maanden met recidiverende luchtweginfecties. Daarnaast heeft patiënt A verslikklachten en een cardiale souffle. Bij echocardiografie bestaat er bij beiden een verdenking op een aortabooganomalie. MRI-onderzoek bevestigt bij beide kinderen het beeld van een incomplete dubbele arcus aortae met een kommerell-divertikel. Na enkele maanden heeft er bij beide patiënten chirurgische correctie van de vaatring plaatsgevonden, sindsdien hebben zij geen klachten meer. De ontwikkeling van anomalieën van de aortaboog en aftakkingen daarvan komen relatief vaak voor en zijn meestal asymptomatisch. Op de kinderleeftijd is de trachea gemakkelijker comprimeerbaar. Deze kinderen presenteren zich vaak met respiratoire symptomen als er vast voedsel wordt geïntroduceerd. Tijdens de volwassen leeftijd is het meer waarschijnlijk dat de oesofagus wordt gecomprimeerd, patiënten presenteren zich dan voornamelijk met dysfagieklachten.
Summary
Within several months, two young children present themselves with recurrent upper respiratory tract infections. Patient A also suffers from dysphagia and a cardiac murmer. When a cardiac ultrasound is performed, there is a suspicion of an aorta anomalia. MRI confirms an incomplete double arcus aortae with a Kommerell’s diverticulum at both children. After several months, both children got a surgical correction of the vascular ring, since then they don’t suffer of any inconvenience anymore. The development of an anomalia of the arcus aortae is relatively common and most of the time asymptomatic. In children the trachea is easily compressed. These children often present themselves with respiratory symptoms when solid foods are introduced. During adulthood the oesophagus is more easily compressed, so that patients most of the time present themselves with symptoms of dysphagia.
