Samenvatting
De indicatie voor een neurochirurgische hersteloperatie bij kinderen met een obstetrisch plexus-brachialisletsel (OPBL) is een parese van de m. biceps < MRC 3 op de leeftijd van 3 maanden. Er wordt gekeken naar elleboogflexie tegen de zwaartekracht. Sommige kinderen kunnen echter ook hun elleboog buigen zonder activiteit van de m. biceps. Een veelvoorkomende truc is de elleboogflexie met behulp van het steindler-effect. Hierbij wordt de elleboog gebogen met behulp van de m. extensor carpi radialis, de hand is dan in pronatiestand.
Doel van het huidige onderzoek was om vast te stellen welk type zenuwletsel (axonotmesis, neurotmesis of avulsie) kinderen hadden die hun elleboog buigen met het steindler-effect. Door middel van statusonderzoek zijn 28 kinderen met een OPBL (geboren 1997-1999) onderzocht die bij bezoek aan het plexusteam in Leiden actieve elleboogflexie lieten zien met behulp van het steindler-effect. Wij vonden dat bij 72% van deze kinderen een ernstig zenuwletsel bestond, waarvoor neurochirurgische reconstructie noodzakelijk was. Goed differentiëren tussen elleboogflexie op basis van de activiteit van de m. biceps en het steindler-effect is belangrijk om vast te stellen of er een ernstig zenuwletsel bestaat, zodat kinderen die neurochirurgische expertise (en soms een zenuwoperatie) nodig hebben tijdig worden herkend.
Summary
The standard indication for a neurosurgical operation in children with obstetric brachial plexus lesion (OPBL) is the absence of activity < MRC 3 of the biceps muscle at the age of 3 months. Clinically this is assessed by observing elbow flexion against gravity. However, some children can bend their elbows without biceps activity by the use of so-called trick movements. A frequently seen trick movement is the Steindler effect. The Steindler effect is flexion of the elbow using the extensor carpi radialis muscle, characteristically with the hand in pronation.
The aim of our study was to determine which type of nerve lesion (axonotmesis, neurotmesis or root avulsion) was present in children who show the Steindler effect. By means of chart review we identified 28 children with OPBL (born 1997-1999) seen at the brachial plexus team in Leiden, the Netherlands, who had shown a Steindler effect. Of these children, 72% had a severe nerve lesion for which neurosurgical reconstruction proved necessary. Good differentiation between elbow flexion based on m. biceps activity and the Steindler effect is crucial to avoid inadequate neurological assessment. This may lead to underestimation of the severity of the nerve lesion in some children, while the indication for neurosurgical nerve repair may be present.
