Samenvatting
Een vierjarig meisje presenteerde zich met een vreemd ademhalingspatroon tijdens de slaap. Ouders beschreven adempauzes tot 20 seconden en progressieve vermoeidheid overdag. Een obstructieve apneu ten gevolge van een vergroot adenoїd werd het meest waarschijnlijk geacht, maar adenotomie bood geen verlichting van de klachten. Polysomnografie was bewijzend voor centrale apneus en MRI-cerebrum toonde een chiari-I-malformatie (CM-I). Patiënte onderging een foramen magnum decompressie met duraplastiek en de nachtelijke apneus verminderden duidelijk. Wij bespreken de differentiaaldiagnose van apneus, de rol van polysomnografie in het vaststellen van het type apneu en de specifieke symptomen van CM-I bij kinderen.
Summary
A four-year-old-girl presented with sleep disordered breathing. Her parents described breathing pauses up to 20 seconds and progressive tiredness during the day. An obstructive apnea from an enlarged adenoid was thought to be the most probable cause. However, an adenotomy did not resolve the complaints. Polysomnography demonstrated central apneas and MRI of the brain revealed a Chiari-I-Malformation (CM-I). A foramen magnum decompression with duraplasty was performed and the amount of apneas during the night obviously decreased. We discuss the differential diagnosis of apneas, the role of polysomnography in determining the type of apnea and specific symptoms of CM-I in children.
