Geïntegreerde eerstelijns dementiezorg met DementieNet

Samenvatting

Zorg voor mensen met dementie die thuis wonen is complex en vraagt om afstemming tussen betrokken eerstelijns hulpverleners. Met DementieNet, een tweejarig programma, faciliteerden we de samenwerking tussen professionals in eerstelijns zorgnetwerken. Netwerksamenwerking en kwaliteit van zorg verbeterden gedurende de 2 jaar. Deze verbeteringen werden in stand gehouden na beëindiging van het programma. Zorgkosten bleven gelijk. Het DementieNet-programma lijkt daarmee een duurzame verandering naar meer geïntegreerde dementiezorg te stimuleren.
