Samenvatting

De patiënt meet thuis met een eigen automatische meter zijn bloeddruk, logt in op het patiëntenportaal Engage van Vital-Health Software en geeft de waardes door. Engage is gekoppeld aan het keteninformatiesysteem (KIS) en de waardes komen automatisch in ons HIS (Medicom) terecht. Binnen een dag krijgt de patiënt terugkoppeling van de CVRM-assistente of POH.
