Op zoek naar het stille hartinfarct

Samenvatting

Om stille en niet-herkende hartinfarcten te voorkomen, pleiten sommige collega’s ervoor laagdrempelig ecg’s te verrichten bij patiënten die plotseling atypische klachten hebben. Daarop valt echter wel wat af te dingen. Er is geen bewijs dat deze aanpak werkt en er valt ook nog geen uitspraak over te doen, omdat diverse gegevens onbekend zijn. Screening is evenmin de oplossing, omdat de mogelijke voordelen niet opwegen tegen de nadelen. De richtlijnen bevelen ecg-screening dan ook niet aan. Daarnaast zijn er praktische bezwaren: hoe selecteer je de hoogrisicopatiënten uit de vele patiënten met atypische klachten?
