Allergische rhinitis en allergische conjunctivitis zijn veelvoorkomende, vaak seizoensgebonden aandoeningen die de patiënt veel ongemak bezorgen. Niet zelden worden verschillende middelen ingezet om de klachten te bestrijden, en het resultaat is wisselend. Er is steeds meer bewijs dat subcutane immunotherapie (SCIT) en sublinguale immunotherapie (SLIT) effectieve en veilige behandelingen zijn van de onderliggende aandoening. SLIT, waarbij de patiënt zelf dagelijks een smelttablet onder de tong legt, heeft daarbij het voordeel dat er geen injectie nodig is.
Immunotherapie is, ook voor de huisarts, zeker het overwegen waard voor patiënten bij wie de klachten ondanks behandeling persisteren. De indicatiestelling en het instellen van een SCIT zijn voorbehouden aan de specialist (allergoloog, kno-arts, kinderarts, longarts); de huisarts kan eventueel de onderhoudsbehandeling overnemen na een goede overdracht. Een SLIT kan echter onder voorwaarden ook door de huisarts zelf ingesteld worden. In deze nascholing worden de effectiviteit van immunotherapie, de indicaties en contra-indicaties, de veiligheid en met name de praktische uitvoering in de huisartsenpraktijk besproken.

Allergic rhinitis and allergic conjunctivitis are common seasonal diseases that cause patients considerable discomfort. Patients are often treated with a number of medications to relieve symptoms, with variable success. There is increasing evidence that subcutaneous immunotherapy (SCIT) and sublingual immunotherapy (SLIT) are safe and effective treatments for the underlying disorder. SLIT, in which the patient takes one sublingual tablet daily, has the advantage that injections are not necessary. Immunotherapy is worth considering in patients with persistent symptoms despite treatment. Whereas only an ENT doctor or allergologist can decide whether SCIT is indicated, GPs can take over responsibility for maintenance therapy, provided they are fully informed about the treatment protocol. In contrast, under certain conditions a GP can initiate SLIT. This article discusses the effectiveness of immunotherapy, its indications and contraindications, its safety, and practical aspects of its use in general practice.
