Depressie komt veel voor en de meeste Nederlanders met een depressie worden in de huisartsenpraktijk behandeld. De onlangs herziene NHG-Standaard Depressie geeft aanbevelingen voor die behandeling. Staan deze aanbevelingen garant voor optimale zorg?
In het kader van de Nederlandse Studie naar Depressie en Angst (NESDA) bleek dat huisartsen een depressieve stoornis meestal redelijk goed herkennen, al is de herkenning minder goed wanneer de patiënt niet zelf met een psychische klacht komt. Huisartsen volgden de criteria van de NHG-Standaard (eerste herziening) bij het verwijzen van patiënten met een depressie: zij verwezen 58% van de patiënten en er was weinig overbehandeling met antidepressiva. De grootste groep overbehandelde patiënten bestaat uit chronische gebruikers die hadden kunnen stoppen.
De tweede herziening van de NHG-Standaard Depressie heeft voldoende aandacht voor diagnostiek en behandeling. Er is echter nog weinig aandacht voor de onderhoudsbehandeling met antidepressiva. De herziene standaard bevat geen advies over de frequentie waarmee chronisch gebruik gecontroleerd wordt en hoe het zo nodig afgebouwd kan worden.

Depression is common and most Dutch people with depression are treated in primary care. The recently revised Guideline on Depression of the Dutch College of General Practitioners (NHG) provides recommendations about treatment, but does this advice guarantee optimal care? The Nederlandse Studies naar Depressie en Angst (NESDA, the Netherlands Study of Depression and Anxiety) showed that general practitioners can usually recognize the disorder, but recognition is less good if the patient does not present mental health problems. GPs followed the NHG guideline (first revision) with regard to the referral of patients with depression: 58% of patients were referred and overtreatment with antidepressants was uncommon. The largest group of over-treated patients consisted of chronic users who should have stopped using antidepressants. While the second revision of the NHG guideline pays enough attention to diagnosis and treatment, it gives too little attention to maintenance treatment with antidepressants. The guideline does not provide recommendations about how often antidepressant use should be monitored among chronic users and how antidepressants should gradually be withdrawn, if necessary.
