Achtergrond
In Midden-Limburg en de westelijke mijnstreek screent men sinds 2008 patiënten met diabetes mellitus type 2 op diabetische retinopathie (DRP) door middel van fundusfotografie. Dit artikel beschrijft een pilotonderzoek naar de kwaliteit van de beoordeling van de fundusfoto’s door speciaal hiertoe opgeleide huisartsen (zogenaamde graders).
Methode
Op basis van fundusfoto’s, vervaardigd tussen januari 2008 en december 2010, hebben we een gestratificeerde selectie van vijftig patiënten gemaakt, bestaande uit twintig foto’s zonder DRP, tien foto’s met matige DRP, vijf foto’s met ernstige DRP en vijf foto’s met overige afwijkingen. Een oogarts heeft deze selectie beoordeeld op de kwaliteit van de foto’s, aanwezigheid van DRP en overige afwijkingen. De beoordelingen van de oogarts en huisarts-graders hebben we met elkaar vergeleken.
Resultaten
Dit pilotonderzoek laat een sensitiviteit van 100% en een specificiteit van 86% zien voor wat betreft het detecteren van DRP: de graders misten de diagnose DRP niet. Overige afwijkingen misten ze echter vaak; de negatief voorspellende waarde hiervoor was 18%. Ook de kwaliteit van de foto’s vormt een punt van aandacht.
Conclusie
Screening van diabetische retinopathie door hiertoe opgeleide huisartsen is goed mogelijk. De kwaliteit van de fundusfoto’s en het signaleren van overige afwijkingen zijn echter voor verbetering vatbaar. De huidige opleiding van graders en assistenten is inmiddels op beide punten aangepast.

Background
Patients with diabetes mellitus type 2 living in mid and south Limburg have been screened for diabetic retinopathy on the basis of fundus photos since 2008. This pilot study investigated how well specially trained general practitioners (so-called graders) detect diabetic retinopathy on fundus photos.
Method
Of all fundus photos taken between January 2008 and December 2010, those of 50 patients were selected: 20 photos without evidence of diabetic retinopathy, 10 photos showing mild retinopathy, 5 photos showing severe retinopathy, and 5 photos showing other abnormalities. An ophthalmologist assessed these photos on the basis of their quality and the presence of retinopathy and other abnormalities, and the evaluations of the graders and the ophthalmologist were compared.
Results
Retinopathy was detected with a sensitivity of 100% and a specificity of 86%. While the graders did not fail to detect retinopathy, they often failed to detect other abnormalities (the negative predictive value was 18%). The quality of the photos was a point of concern.
Conclusion
Specially trained GPs can detect diabetic retinopathy; however, the quality of fundus photos and the detection of abnormalities other than retinopathy could be improved. The training programme for GP graders and practice assistants has since been adapted to address these aspects.
