Patiëntgerichtheid is vanuit ethisch perspectief een doel op zich. Artsen moeten hun zorg zo leveren dat de inhoud en vorm in overeenstemming zijn met de waarden en voorkeuren van de individuele patiënt. Dat geldt zeker bij zogenaamde preferentie- of optiegevoelige beslissingen, waarbij er meer dan één behandeloptie is of waarbij er dilemma’s bestaan door een weging van vooren nadelen. Dat vraagt om een dialoog met patiënten. Patiënten hebben ondersteuning nodig in die dialoog. Zogenaamde keuzeof beslishulpen voor patiënten lijken in deze behoefte te kunnen voorzien. In deze nascholing bespreken we de volgende vragen en aspecten. Wat zijn keuzehulpen? Wat is het doel van keuzehulpen? We beschrijven de historie en recente ontwikkelingen. Hoe moeten ze worden gebruikt? In de spreekkamer van de huisarts, bij wie (niet), en hoe dan? Zijn er keuzehulpen beschikbaar in Nederland? We geven enkele voorbeelden van keuzehulpen. Wat is de toekomst voor keuzehulpen?
Keuzehulpen voor patiënten geven de patiënt inzicht in de beschikbare opties, de mogelijke consequenties van iedere optie en de manier waarop hij de consequenties van iedere optie waardeert (preferentieconstructie). De patiënt kan desgewenst actief deelnemen aan de besluitvorming. Keuzehulpen kunnen gedeelde besluitvorming effectief ondersteunen en verhogen de kwaliteit van het besluitvormingsproces. De verwachting is dat keuzehulpen voor patiënten geïntegreerd zullen worden in de besliskundige instrumenten voor artsen, zoals klinische praktijkrichtlijnen.

From an ethical perspective, patient-centeredness is a goal in itself. Doctors should provide care in such a way that its form and content are compatible with the values and preferences of individual patients. This is particularly true for so-called preference or option-sensitive decisions, where there is more than one treatment option or when there are dilemmas in making the balance between the advantages and disadvantages of the different options. This needs to be discussed with patients, who often need support when making decisions. Such support can be provided by patient decision aids. These instruments are discussed in this article. What are decision aids? What is their function? The history of these instruments and recent developments are described. How should they be used – in the doctor’s office, how and by whom? Are decisions aids available in the Netherlands? Examples are given. What is the future of decision aids?
Patient decision aids provide patients with insight into available treatment options, the potential consequences of each option, and the way in which these consequences should be weighed (preference construction). If desirable, patients can have an active role in decision-making. Decision aids can effectively support shared decisionmaking and improve the quality of the decision-making process. It is expected that patient decision aids with be incorporated in decision-making instruments for doctors, such as clinical guidelines.
