Doel
Wij hebben onderzocht welke risico’s voor de patiëntveiligheid naar voren kwamen uit medische tuchtzaken bij regionale tuchtcolleges tegen huisartsen.
Methode
Wij analyseerden 250 uitspraken van regionale tuchtcolleges, gepubliceerd tussen 2008 en 2010, en richtten ons daarbij vooral op tuchtklachten waarbij de betrokken patiënten ernstige gezondheidsschade hadden geleden.
Resultaten
Van de 74 ernstige klachten (blijvende schade of overlijden) betroffen er 33 (45%) een verkeerde diagnose, 17 (23%) onvoldoende zorg, 6 (8%) een verkeerde behandeling, 6 (8%) te late aankomst bij een visite, 4 (5%) te late verwijzing naar het ziekenhuis, 1 (1%) onvoldoende informatie en 7 (10%) andere oorzaken. De verkeerde of (te) late diagnoses betroffen meestal hartinfarcten of CVA’s (35%) en maligniteiten (34%). De helft van alle tuchtklachten werd gegrond verklaard; bij ernstige klachten was de kans op gegrondverklaring groter dan bij minder ernstige.
Conclusie
Incidenten met ernstige gezondheidsschade voor de betrokken patiënt komen voor een aanzienlijk deel voort uit het missen van een potentieel ernstige ziekte zoals een hartinfarct, CVA of maligniteit. Een betere diagnostiek van deze aandoeningen kan mogelijk bijdragen aan de patiëntveiligheid.
