Een conversiestoornis is een psychiatrische aandoening die zich uit in neurologische klachten die kunnen variëren van milde tot ernstige motorische en/of sensorische uitvalsverschijnselen. Op de huisartsenpost wordt het stellen van de diagnose conversiestoornis en het uitsluiten van een somatische oorzaak bemoeilijkt door het kortdurende contact met een onbekende patiënt met vaak acute klachten. Een conversiestoornis wordt geassocieerd met conflicten of andere stressveroorzakende factoren. Bij het vermoeden van een conversiestoornis is het uitvragen van de psychische toestand en eventuele stressoren dan ook zinvol. Het inzien van de medische gegevens uit het dossier van de eigen huisarts kan de dienstdoende huisarts extra informatie verschaffen. Bij het lichamelijk onderzoek onderscheidt een conversiestoornis zich van acute somatische aandoeningen door afwijkingen die variëren, inconsistent zijn en/of niet zijn te objectiveren. Desondanks is het voor de dienstdoende huisarts moeilijk om een conversiestoornis met zekerheid te onderscheiden van een somatische aandoening. Bij twijfel over de diagnose, hevige ongerustheid en/of angst bij patiënt of naasten is verwijzing naar het ziekenhuis geïndiceerd. Als de dienstdoende huisarts besluit de patiënt niet in te sturen, vormen een korte uitleg over de oorzaak en het beloop en een goede terugkoppeling aan de eigen huisarts een belangrijke basis voor verdere behandeling.

Conversion disorder is a psychiatric condition that presents with neurological symptoms ranging from mild to severe motor and/or sensory symptoms or deficits. In afterhours general practice clinics, it is difficult to establish a diagnosis of conversion disorder with exclusion of somatic causes because of the short consultation with an unknown patient who often presents with acute symptoms. Conversion disorder is associated with conflict or other stressful factors, and if suspected, it is worthwhile asking about the patient’s psychological condition and possible stressors. The patient’s medical record might also provide additional information. On physical examination, conversion disorder can be distinguished from acute somatic illnesses by symptoms or deficits that are variable and inconsistent, or that cannot be objectified. Even so, it is difficult for the general practitioner on duty to distinguish accurately between conversion disorder and somatic illnesses. Referral to hospital is indicated if there is doubt about the diagnosis or if the patient or relatives are severely agitated or anxious. If the patient is not referred to hospital, a brief explanation of the cause and course of symptoms and good feedback to the patient’s own doctor form an important basis for further treatment.
