Invloed van focusgroepen op NHG-Standaarden en implementatieproducten

Achtergrond Om NHG-Standaarden en bijbehorende implementatieproducten beter af te stemmen op het gebruik door huisartsen, organiseert het NHG tijdens de ontwikkeling daarvan groepsinterviews (‘focusgroepen’), waarin de huisartsen meepraten over de inhoud en ook de implementatie van de standaard. Wij wilden weten in hoeverre de adviezen van deze huisartsenfocusgroepen ook daadwerkelijk worden overgenomen en zo nee, waarom niet. Ook wilden we weten of de deelnemers tevreden waren over de bijeenkomsten.

Methode Voor zeventien NHG-Standaarden gingen wij na hoeveel van de adviezen van focusgroepen terug te vinden was in de standaarden en implementatieproducten, en vroegen wij de NHG-medewerkers naar de redenen om een advies af te wijzen. De tevredenheid van de deelnemers maten wij met een vragenlijst onder deelnemende huisartsen en een groepsinterview onder NHG-medewerkers.

Resultaten Van de in totaal 281 inhoudelijke adviezen die de zeventien focusgroepen gaven, werden er 209 (74%) overgenomen in de definitieve teksten van de standaarden. Redenen om een advies af te wijzen, waren meestal een verschil van opvatting over de inhoud of over het belang ervan. Van de 151 voorgestelde implementatieproducten – voor het oplossen van 94 implementatieknelpunten – werden er 93 (62%) daadwerkelijk gerealiseerd. Naast het ingeschatte belang speelde hier ook een rol of het voorgestelde product al dan niet paste in het bestaande arsenaal. Zowel de NHG-medewerkers als de responderende huisartsen waren zeer tevreden over de focusgroepbijeenkomsten.

Conclusie Groepsinterviews met huisartsen zijn waardevol voor het beter afstemmen van de inhoud van de NHG-Standaarden en implementatieproducten op de wensen van praktiserende huisartsen.

Abstract

Introduction Focus group meetings are a way to elicit the input of general practitioners regarding the content and implementation of Nederlands Huisartsen Genootschap (NHG; Dutch College of General Practitioners) Guidelines. The aims of this study were to determine the extent to which the advice of these focus groups is implemented, the reasons for not implementing the advice, and how satisfied participants are with these meetings.

Method The extent to which the advice of focus groups was incorporated into 17 NHG Guidelines and educational products and materials for doctors and patients was investigated, as were the reasons why the advice was not implemented, where relevant. Satisfaction was measured by means of a questionnaire sent to participating general practitioners and a group interview with NHG staff.

Results The 17 focus groups made 281 recommendations regarding the content of the NHG Guidelines, of which 209 (74%) were incorporated in the final text. The main reasons for ignoring recommendations or advice were differences of opinion about the content and relevance of the advice. Of the 151 educational products and materials proposed (for resolving 94 bottlenecks to implementation), 93 (62%) were effectuated on the basis of their estimated importance and compatibility with existing educational products and materials. Both NHG staff and general practitioners were very satisfied with the focus group meetings.

Conclusion Focus group meetings with general practitioners are valuable for adjusting the content of NHG Guidelines and recommendations to the requirements of general practitioners. Whether this leads to better guideline compliance has yet to be ascertained.
