Een charcotvoet is een zeldzame complicatie bij patiënten die langer dan tien jaar diabetes mellitus hebben. Directe herkenning en onmiddellijke immobilisatie kunnen ernstige schade aan de voet voorkomen, maar vaak wordt aanvankelijk een verkeerde diagnose gesteld. De pathogenese van de charcotvoet wordt steeds duidelijker: bij alle patiënten ziet men neuropathie met verlies van beschermende sensibiliteit. Doordat de patiënt niets voelt en de voet blijft belasten, kan een kleine verwonding een cascade op gang brengen van ontstekingsverschijnselen en botafbraak. Meestal is de middenvoet aangedaan. De voet is gezwollen, rood, warm en enigszins pijnlijk. Op de röntgenfoto zijn in het beginstadium meestal geen afwijkingen te zien; de diagnose kan worden gesteld met MRI, CT of PET. Onmiddellijke gipsimmobilisatie is aangewezen. Na het verdwijnen van de roodheid en zwelling kan de patiënt onder nauwgezette controle de voet gaan belasten met aangepast schoeisel. Bij te late herkenning raakt de voet in korte tijd volledig misvormd.
Casus
De heer A. is 70 jaar en heeft sinds twintig jaar type-2-diabetes. Hij is onder behandeling van de internist en volgt een schema van viermaal daags insuline. Begin 2009 krijgt hij zonder directe aanleiding een rode, gezwollen en pijnlijke rechter enkel. Op de röntgenfoto zijn geen afwijkingen te zien. In de loop van dat jaar krijgt hij meer klachten en ontstaat een standafwijking van de enkel. Pas na ruim een jaar stelt een orthopeed de diagnose charcotvoet en volgt behandeling met gipsimmobilisatie. Na vier maanden kan de voet langzaam weer zwaarder worden belast. Gelukkig blijven de gevolgen beperkt tot een licht afwijkende stand van de enkel en kan de heer A. nog steeds lopen, zij het met behulp van orthopedische schoenen.

The Charcot foot is a rare complication of diabetes mellitus seen in patients who have had diabetes for more than 10 years. Al though prompt recognition and direct immobilization are important to prevent serious damage to the foot, Charcot foot is often initially misdiagnosed. The pathogenesis of the Charcot foot is becoming clearer: all patients with Charcot foot have neuropathy with loss of protective sensation. Because the patient does not feel anything, he/she continues to use the foot, so that even a minor injury can lead to activation of the inflammatory cascade and bone breakdown. The symptoms of Charcot foot are a warm, red, swollen, and tender foot; the mid-foot is most often affected. In the early stages abnormalities are usually not seen on standard X-rays, but the diagnosis can be established with MRI, CT, or PET. The foot should be immediately immobilized in a non-weight bearing total contact cast. After symptoms have resolved, the patient can start to put weight on the foot. This should be done under close supervision and an adapted or customized shoe should be used. Delayed recognition of Charcot foot can rapidly lead to severe foot deformation.
