Achtergrond
Het aantal recepten voor een SSRI is in Nederland de laatste jaren sterk gestegen. Wij zijn nagegaan voor welke indicaties huisartsen een SSRI voorschrijven, hoe lang de SSRI-behandeling duurt en hoe de zelfgerapporteerde therapietrouw is.
Opzet en methode
Wij onderzochten de gegevens over 2006 van 11.951 patiënten in 7 huisartsenpraktijken in Diemen. Wij verkregen deze gegevens via de apotheek, uit het elektronisch medisch dossier (EMD) en uit telefonische interviews of schriftelijke enquêtes.
Resultaten
In de onderzoeksperiode gebruikten 249 geïncludeerde patiënten, vooral vrouwen van middelbare leeftijd, een SSRI. In de meeste gevallen (68,3%) was dat paroxetine. Depressie en angst waren de voornaamste indicaties, maar 47% van de recepten betrof off-labelgebruik voor een nietofficiële indicatie. Meer dan tweederde (70%) van de patiënten gebruikte hun antidepressivum al langer dan één jaar, en ruim 80% zei de medicatie volgens voorschrift te gebruiken.
Conclusie
Huisartsen volgen in hun keuze van een SSRI grotendeels de richtlijnen van het NHG. Zij wijken echter in bijna de helft van de voorschriften af van de officieel geregistreerde indicaties, en de gebruiksduur is in de praktijk vaak langer dan de richtlijnen adviseren.

Aim
There has been a strong increase in the number of prescriptions for serotonin re-uptake inhibitors (SSRIs) in recent years in the Netherlands. We investigated the indications for which general practitioners prescribed SSRIs, the duration of treatment, and self-reported treatment compliance.
Method
We collected data for 2006 from dispensing chemists, electronic patient files, telephone interviews and questionnaires regarding 11,951 patients from seven general practices in Diemen, the Netherlands.
Results
In 2006, 249 of the included patients, mainly middleaged women, used an SSRI, mainly paroxetine (68.3%). The main indications were depression and anxiety, but 47% of the prescriptions were off-label for non-approved indications. Seventy percent of the patients had taken antidepressants for more than 1 year, and more than 80% said they used medications as prescribed.
Conclusion
General practitioners mainly follow the recommendations of the Dutch College of General Practitioners when prescribing an SSRI. However, nearly half of the prescriptions were for non-approved indications and the duration of use was often much longer than that advised by the guidelines.
