Niet-traumatische knieklachten bij volwassenen: prognose en beleid

Om de prognose van niet-traumatische knieklachten te bepalen includeerden we patiënten ouder dan 35 jaar die de huisarts consulteerden vanwege niet-traumatische knieklachten en volgden hen één jaar. Bij de start van het onderzoek verzamelden we gegevens over de knieklachten, demografische kenmerken en het beleid van de huisarts. Ook voerden we bij de aanvang van het onderzoek en na één jaar follow-up een lichamelijk onderzoek uit. Aan de hand van driemaandelijkse vragenlijsten volgden we gedurende één jaar de knieklachten en de medische consumptie. Na één jaar gaven 236 patiënten (49%) aan persisterende knieklachten te hebben. De belangrijkste voorspellers voor persisterende knieklachten waren de klachtkarakteristieken (voorgeschiedenis van niet-traumatische knieklachten, bilaterale klachten, duur van de klachten > 3 maanden). Factoren van lichamelijk onderzoek hadden geen enkele voorspellende waarde voor persisterende knieklachten. Ook de klinische ACR-classificatiecriteria voor gonartrose bleken geen voorspellende waarde te hebben.

Wat betreft het initiële medische beleid adviseerde de huisarts 193 patiënten (35%) de knie te ontzien en kregen 150 (27%) pijnmedicatie voorgeschreven. In totaal kregen 311 patiënten (57%) een verwijzing naar de fysiotherapeut, orthopeed of het ziekenhuis voor een röntgenfoto van de knie. Het medische beleid kwam gedeeltelijk overeen met de aanbevelingen uit de NHGStandaard Niet-traumatische knieproblemen bij volwassenen. Ook was de medische consumptie gedurende één jaar follow-up relatief hoog.
